1. Medical Services

SoCal Men’s Clinic provides medical consultations, wellness evaluations, hormone optimization services, erectile dysfunction treatment, weight management support, laboratory review, preventative care services, and related healthcare services where permitted by law.All medical care is provided at the discretion of a licensed medical provider based on clinical judgment, medical history, laboratory findings, and patient eligibility.Submission of forms, payment, or scheduling an appointment does not guarantee treatment, prescriptions, medication approval, or establishment of a provider-patient relationship.

2. Telehealth Services

Patients may receive services through telemedicine platforms when appropriate and permitted under applicable state and federal laws.By participating in telehealth services, you acknowledge and agree that:

  • Telehealth has limitations compared to in-person evaluations.
  • Technical issues may occasionally disrupt appointments.
  • Providers may recommend in-person evaluation, emergency care, or additional testing when medically necessary.
  • Not all conditions are appropriate for telemedicine treatment.

Patients are responsible for ensuring they are physically located in a state where the provider is authorized to practice at the time of the appointment.

3. Patient Responsibilities

Patients agree to:

  • Provide accurate and complete medical information.
  • Update the clinic regarding changes in medications, symptoms, or medical history.
  • Complete required laboratory testing when requested.
  • Follow prescribed treatment plans and provider instructions.
  • Attend scheduled appointments or provide appropriate cancellation notice.
  • Use medications only as prescribed.

Failure to comply with treatment recommendations, follow-up appointments, laboratory requirements, or clinic policies may result in delayed treatment, suspension of services, or discontinuation of care.

4. Membership & Subscription Services

Certain services may be offered through recurring membership plans or subscription-based care programs.By enrolling in a membership program, you understand and agree that:

  • Membership fees may recur monthly unless canceled.
  • Membership pricing, services, and benefits may change with notice.
  • Membership fees generally cover provider oversight and designated services only.
  • Certain medications, specialty laboratory tests, supplies, or outside services may require additional charges.
  • Missed appointments do not automatically qualify for refunds.

Patients are responsible for maintaining a valid payment method on file.

5. Payments & Billing

Payment is due at the time services are rendered unless otherwise stated.By providing payment information, you authorize SoCal Men’s Clinic to charge your card or payment method for:

  • Consultation fees
  • Membership fees
  • Laboratory fees
  • Administrative fees
  • Medication-related charges when applicable
  • Late cancellation or no-show fees

Outstanding balances may result in delayed appointments, suspension of services, or collection activity where permitted by law.

6. Refund Policy

Medical services, consultations, completed visits, administrative work, and laboratory services are generally non-refundable.Medication orders that have already been processed, shipped, compounded, or dispensed cannot be returned or refunded unless required by law.Refund requests are reviewed on a case-by-case basis at the discretion of the clinic.

7. Prescription Policies

Prescription decisions are based solely on medical necessity and provider judgment.The clinic does not guarantee approval of testosterone therapy, erectile dysfunction medication, peptide therapy, weight loss medication, or any other treatment.Patients understand that:

  • Certain medications may require ongoing monitoring.
  • Laboratory testing may be required before and during treatment.
  • Controlled substances are regulated by federal and state law.
  • Prescriptions may be delayed, modified, or denied based on clinical findings or safety concerns.

8. Laboratory Testing

Laboratory testing may be required before treatment initiation and throughout care.Patients are responsible for completing required laboratory work within requested timeframes. Failure to complete required testing may result in delayed refills, paused treatment plans, or discontinuation of services.

9. Privacy & HIPAA

10. Communication Consent

By submitting forms or providing contact information, you consent to receive communications from SoCal Men’s Clinic, including:

  • Phone calls

Message and data rates may apply. Patients may opt out of non-essential communications at any time.

11. Cancellation & No-Show Policy

Patients are encouraged to provide at least 24 hours’ notice for cancellations or appointment changes.Missed appointments or late cancellations may result in fees or temporary scheduling restrictions.Repeated no-shows or noncompliance may result in discharge from the practice.

12. Medical Emergencies

SoCal Men’s Clinic does not provide emergency medical services.If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.Do not rely on electronic messages, emails, or telehealth scheduling systems for urgent or emergency medical needs.
