Complete Your Intake Forms

Fill out your new patient forms using the link above. Once submitted, you’ll receive an invitation to join our patient portal — please create your account, as this is how we’ll communicate with you throughout your care.

Complete Your Labs

After your forms are reviewed, we’ll send your lab requisition through the portal. Take it to any LabCorp location — no appointment needed. Most results are back in 1–3 days.

We'll Schedule Your Visit

Once your lab results are in, our team will reach out with a link to book your telehealth appointment with Dr. Gerson. You’ll receive everything you need to join the video call.
